About MAM Care Services:
MAM Care Services Ltd is a specialist complex care provider delivering compassionate, skilled support to individuals with disabilities and complex medical needs in their own homes.
Founded on both professional expertise and lived experience, MAM Care was created to address the gaps that families and individuals often face when trying to access reliable, flexible and high-quality care.
While the organisation initially focused on supporting children with complex needs, the service has developed to support children, young people and adults who require specialist care in the community.
MAM Care works with individuals with a wide range of conditions including physical disabilities, neurological conditions, life-limiting illnesses, learning disabilities and complex medical needs.
Our goal is simple:
- to enable individuals to live safely, comfortably and with dignity in their own homes while receiving the highest standard of care.
- We work closely with families, community healthcare teams, commissioners and case managers to provide responsive, flexible care solutions tailored to each individual.
Experts by Experience:
MAM Care was founded by individuals who understand complex care first-hand.
The organisation was created by Sanam Khan and Rebecca Brunt, who witnessed the challenges families face when trying to build reliable care teams and navigate complex healthcare systems.
Sanam is a mother of three children with severe disabilities and complex medical needs and has over 13 years of lived experience managing complex care and Personal Health Budgets. Through managing her own children's care team and coordinating specialist medical care, she gained invaluable insight into the realities families face every day. This experience became the driving force behind the creation of MAM Care.
Rebecca previously worked as a personal care assistant supporting Sanam’s family and witnessed first-hand the gaps in care provision and the pressures placed on families.
Together they created MAM Care to ensure that families and individuals receive the practical, flexible support that they genuinely need, rather than being forced into rigid care models. This “Experts by Experience” philosophy shapes every aspect of the organisation.

SEND Support and Preparing for Adulthood:
MAM Care also works with families navigating the complex systems surrounding Special Educational Needs and Disabilities (SEND).
Children and young people with SEND often require coordinated support across health, education and social care services.
Transition planning for adulthood typically begins during early adolescence and continues through young adulthood, particularly for those with an Education, Health and Care Plan (EHCP).
In England, preparation for adult services can begin around age 14, with a gradual transition to adult services usually taking place around 18 years of age, although support through education services may continue until age 25 for young people with an EHCP.
MAM Care supports families during this important transition period by helping them maintain consistent care while navigating the changes between children’s and adult services.
Our experience in complex care and Personal Health Budgets allows us to guide families through:
-Transition planning for adulthood
-Understanding available funding options
-Building and managing care teams
-Maintaining continuity of care during service changes
This support helps prevent the “cliff-edge” situation many families face when young people reach adulthood and services suddenly change.
Supporting Personal Health Budgets (PHB):
MAM Care goes beyond providing care services. We also support families and individuals who are managing Personal Health Budgets (PHBs).
A PHB allows individuals to take greater control over how their healthcare is delivered and who provides it.
Our team can help families:
-Understand the PHB process
-Access relevant information and resources
-Build effective care teams
-Manage staffing and care planning
By supporting families to manage their own care packages, we empower them to make decisions that best suit their needs and circumstances.
