Humans Cornwall provides personalised holistic support to individuals who are transitioning back home from the hospital. Our mission is to ensure that our clients receive the care and support they need to make a successful recovery and regain their independence.

Our services include a range of support options, tailored to meet each client's unique needs. We offer practical help carefully coordinated by our support brokerage team, including everything from care provision, accessing grants to purchase essential items or aids and adaptions, welfare benefits support, and anything else that matters to a person.

Our team of dedicated professionals includes Personalised Care Leads, Reablement PAs, a bank of Personal Lifestyle Assistants, and a team of Micro Providers who we work collaboratively with across Cornwall, all who work closely with each client to provide a holistic approach to care.

At Humans Cornwall, we understand that the transition from hospital to home can be challenging, both physically and emotionally. That's why we're committed to providing compassionate, individualised care and support to each of our clients. Our goal is to help our clients regain their strength and confidence, so they can get back to doing the things they love.

We work collaboratively with health and social care colleagues, community Teams including private care agencies, occupational therapy to palliative cancer teams to stabilise and coordinate services and support around a person.

The Humans Cornwall model of support is leading the way for personalisation in Cornwall, and we have worked successfully with other integrated care areas to share ideas, learning and our model.
